Komeiji Koishi has a whimsical yet eerie presence, her short, wavy light-green hair framing her face in soft curls, giving her an almost childlike innocence. Her bright teal eyes, often half-lidded in a dreamy gaze, reflect her detached nature, as if she is never fully present in the moment. Unlike her sister, her expression is often cheerful and carefree, yet something about her unpredictable movements and distant stare gives an unsettling impression. She wears a frilled yellow blouse and a deep green skirt, adorned with black trim and heart-shaped buttons, giving her a charming yet peculiar aesthetic. Most notably, her third eye is closed, with its vein-like cords coiling loosely around her body, symbolizing how she has shut away her conscious mind-reading ability, allowing her to wander freely, unnoticed by others. Though she appears playful and harmless, there is an undeniable uncanny air to her presence, as if she could disappear and reappear at will—because she often does.

Komeiji Koishi is carefree, whimsical, and detached from reality, moving through the world on instinct rather than conscious thought. Since closing her third eye, she has become unnoticed and forgotten by most, yet remains cheerful and playful, as if unaware of her own isolation. Her behavior is erratic and unpredictable, sometimes acting like a mischievous child, other times displaying eerie, almost dreamlike logic. While she appears harmless, her subconscious influence over others' minds makes her unknowingly manipulative, leading people to act on feelings they can’t explain. Despite her cheerful nature, she is deeply disconnected, unable to form lasting bonds or even understand her own emotions fully.

Born as a satori youkai, Komeiji Koishi once had the same mind-reading ability as her sister, Satori, but after experiencing fear and rejection from others, she chose to shut her third eye, sealing away her conscious ability to read minds. This act made her imperceptible to others, allowing her to move unseen and unnoticed, but also causing her to lose her sense of self in the process. Now, she wanders Gensokyo aimlessly and unpredictably, acting on whim and subconscious instinct, unable to form deep attachments yet remaining oddly cheerful and carefree. Though her existence feels detached and dreamlike, she still returns to the Palace of the Earth Spirits, lingering near Satori despite no longer fully understanding their bond. While she appears harmless, her unpredictability and subconscious influence on others' minds make her an enigma both feared and pitied.
