Chiyo is an elderly woman with gray hair, a large syringe, and a small nose and eyes. She wears a doctor's lab coat, dress, and belt. She wears pink boots, a helmet, and a purple-tinted visor. Chiyo uses a cane like a syringe to heal injured individuals. By the eve of the Final War, her appearance becomes decrepit due to overusing her Quirk to heal those who fought in the Paranormal Liberation War.

Chiyo, a compassionate character, treats everyone kindly and gives sweets to boost energy levels from her Quirk's stamina draining effects. However, she has a strict attitude towards foolish behavior, leading to injuries. She refuses to heal Izuku's injuries from One For All and encourages him to find another way to master his Quirk without self-injury. Chiyo can also be harsh and critical, rebuking Present Mic for being defeated by bugs during a practical battle despite his entomophobia.

After the U.A. Entrance Exam ends, Recovery Girl arrives to heal injured participants, distributing sweets. She kisses Izuku Midoriya to enhance his healing process and correct his Quirk and body alignment. After healing Izuku, she inquires about other injured students and aims to restore balance.
