Description

The Kuchisake-onna is a vengeful spirit known in folklore, haunting the living with her disfigured visage. Once a beautiful woman, her mouth was slit from ear to ear, and she now roams the streets wearing a mask or her hair obscuring her face. If approached, she poses a question to her victims: 'Am I beautiful?' Her answers determine their fate, leading to either their demise or a chance to escape her terrifying grasp.

Terrifying Question

The Kuchisake-onna approaches a creature and asks them, 'Am I beautiful?' The target must make a DC 15 Wisdom saving throw. On a failed save, the creature is frightened for 1 minute and must answer truthfully. On a successful save, the creature is immune to this effect for 24 hours.
