The Lands Between

M
Muttboy

The world of Elden Ring, a hauntingly beautiful yet desolate land of gold that is scarred from war.

Author's Note: Still have a ton of work to do -Still trying to figure out how to work casting into classes. -Slowly adding items, monsters, and NPCs -All 307 base game weapons and 171 base game spells added. added, they will still require balancing but waiting for the combat overhaul before I dig too deeply into that. See the following document for homebrew rules specific to this world. https://docs.google.com/document/d/1t7MkfUj7CvzkERJoJGxCYYEd5H6lBEml9pYQl9rHUUY/edit?usp=drivesdk

Wormface
Wormface
Large Aberration - CR 14
Wormface are towering, emaciated creatures notable for their worm-like tentacles that protrude grotesquely from their faces. They are often found in the woodlands of the Altus Plateau and the ruined expanses of Crumbling Farum Azula. These creatures are infamous for their ability to generate clouds of Death Blight, which suffocate and weaken those caught within. Smaller variants can be seen wielding ornate wooden batons and are sometimes draped in faded yellow cloaks. Wormface engage in combat by grabbing their victims and sucking the life force from them with their twisted maws before violently spitting them back out.

Appearance

Tall, thin, and worm-like with tentacles instead of a proper facial structure, their bodies are covered in sickly hues, contrasting with the faded elegance of their cloaks and batons.

Actions

Death Blight Exhalation

Wormface coughs up a cloud of harmful Death Blight in a 30-foot radius around itself. Creatures within this area must make a Constitution saving throw or take 4d10 necrotic damage and become poisoned for 1 minute.

Life Siphon

Wormface attempts to suck a creature into its maw. The target must make a Dexterity saving throw or take 3d10 necrotic damage, and the Wormface regains hit points equal to half the necrotic damage dealt.

Tentacle Grab

Wormface uses its tentacles to grab a creature within 10 feet. The target must succeed on a Strength saving throw or be grappled. While grappled, the victim takes 2d8 necrotic damage at the start of each of Wormface's turns.
