Description
Towering, emaciated creatures with worm-like tentacles protruding from their faces, the Giant Wormface embodies a grotesque and nightmarish presence. They regularly cough up clouds of Death Blight which cover a large area around them, instilling despair in those who encounter them. Predominantly found in the woodlands of the Altus Plateau, some of these dreadful beings have made their homes in Crumbling Farum Azula. The smaller variety, often seen with ornate wooden batons and faded yellow cloaks, perpetuates their eeriness as they grab unfortunate victims to suck on before spitting them out. The colossal variants favor sheer brute force rather than weaponry, utilizing violent headbutts, powerful grabs, and their Death Blight abilities in combat.

Death Blight Breath
The Giant Wormface exhales a cloud of toxic blight in a 30-foot radius, forcing creatures in the area to make a DC 16 Constitution saving throw or take 45 (10d8) necrotic damage and become poisoned for 1 minute. A poisoned creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

The Giant Wormface targets one creature within 10 feet of it, making a melee attack. On a hit, the target is grappled (escape DC 20). Until this grapple ends, the target is restrained, and the Wormface can't grab another target.
Headbutt
